      Looking at businesses in that building it seems like they use Indianapolis and Carmel as their city interchangeably. I would argue the Indianapolis address is simply incorrect and you would put Carmel on your 4473 instead.

      That sounds like a rare exception, the vast majority of the time the case is that you would have an address in a city (based on the zip code) but actually live in an unincorporated area.

      Inside city limits the correct agency is the city police department, outside of city limits the correct agency if the county sheriff's office. This way they'll know which to contact by just looking at the 4473 and not having to look up where the city limits are on a map.
