I was having a little range day with my brothers yesterday. And we were shooting my RRA AR15. The very first time I shot it, I had a FTF. I performed the TAP/RACK/BANG! method. Nothing, and I noticed the cartridge never ejected. So I ejected the mag, racked the charging handle again. And I had 2 cartridges fly out.
I picked them up, and one was good. The other, well, not so good. It was a factory Rem. that I bought from Wal-Mart. And the crimping on it was just horrible. The bullet was in the casing, but it was way inside, with powder just pouring out, when I turned it upside down.
I took some pictures.
