I spoke with the ATF again today. Dudash is on vacation which explains why she didn't call me back. I spoke with a specialist and here are the results. (1) Never use white out. He specifically stated this was a no no. If you need to make a correction, single line through it and write it next to it. (2) Your engraving must be whatever you put in block 1. If you are doing this as an Individual, it must me your full name.
At least I know all of this for the future. I'm not sure if this is something that they are just now starting to enforce or what. For everyone's reference in the future, whatever name you use in block 1 must be what you engrave on the weapon, and even though you don't have to put anything in 4h, do not use abbreviations other than accepted postal abbreviations.
I will be going for some new engraving.
I had the exact issue with my Form 1 I sent in to Nikki last year. I was able to talk to her the first time I called and she laid it out for me black and white....The short version is:
Leave 4H blank.
She said a single line through the error with your initials buy it is the way to go.
The good news is you should have a stamp in your possession within a week of sending it back as it goes on the TOP of the pile when it goes back in.

They have the power to give a variance and they should in situations like this. Look at Thompson Center Contenders, they are marked TCA for the manufacturer.

They do, but in the case of a regular mfg, don't you have to apply for that variance?

Alright, had my stamp come back on Christmas eve, at least that was the day we got it out of the box. It may have been there from the day before. In my case, it did require the entire name on it. Next question, does anyone recommend an engraver?
